The non-participating parent understands they will be required to sign a separate acknowledgement before their child may attend the Focus retreat. Please type YES.

Participants under age 18, but not younger than age 16, may attend with proof of age and with an accompanying parent. The parent must share a double-occupancy room with the participant and is responsible for the participant. The parent may not participate in the Focus retreat.
